Answer Sheets
Answer Sheets for Tests, Surveys, and Evaluations. Tests designed for a single correct answer (multiple-choice, true-false, matching items) can be scored by computer if they are on scanner answer sheets (#4521 or 6703). Form #MB01 can be used for evaluations or surveys only. The following NCS trans-optic mark sense forms (answer sheets) are available from the Examination Services at cost. You can also request answer sheets online.
